1.Comparison of Outcomes of KOH/IUI and IVF Treatment in Advanced Age Infertile Women

GİRİŞ ve AMAÇ: Çalışmada 38 yaş üzeri infertil kadınlarda KOS/ IUI ile IVF tedavilerinin sonuçlarının karşılaştırılması amaçlanmaktadır.
YÖNTEM ve GEREÇLER: 38 yaş üzerindeki KOS/IUI veya IVF tedavisi alan toplam 113 infertil kadın bu retrospektif çalışmaya dahil edilmiştir. Çalışmada KOS/IUI tedavisi uygulanmış 42 hasta, IVF tedavisi uygulanmış 71 hasta bulunmaktadır. İki grup biyokimyasal gebelik, klinik gebelik ve devam eden gebelik sonuçları açısından karşılaştırıldı.
BULGULAR: İki grup arasında biyokimyasal gebelik ve klinik gebelik oranları arasında istatistiksel olarak anlamlı düzeyde farklılık saptandı. Biyokimyasal gebelik ve klinik gebelik oranları IVF tedavisi alan grupta daha yüksekti (p1=0.033, p2=0.031). Devam eden gebelik oranı IVF tedavisi alan grupta daha yüksek olmasına rağmen iki grup arasında istatistiksel derecede anlamlı bir fark saptanmadı (p=0.084).
TARTIŞMA ve SONUÇ: İleri yaş grubundaki hastalarda IVF tedavisi KOS/IUI tedavisinden daha etkin olması nedeniyle bu yaş grubundaki hastalarda ilk tedavi seçeneği olarak IVF tedavisinin uygulanabileceğini düşünmekteyiz.
INTRODUCTION: The aim of the study was to compare the outcomes of patients > 38 years of age who underwent COH/IUI and those who underwent IVF treatment.
METHODS: 113 patients who meet the inclusion criteria were enrolled into the study. Of the 113 patients; 42 patients who underwent KOH/IUI and 71
patients who underwent IVF treatment. The biochemical, clinical and ongoing pregnancy rates were compared between two groups.
RESULTS: There were significant differences in terms of biochemical and clinical pregnancy rates between two groups and those were higher in IVF treatment (p1=0.033, p2=0.031). Ongoing pregnancy rate was higher in IVF treatment group but there was no significant difference between two groups (p=0.084).
DISCUSSION AND CONCLUSION: IVF treatment may be first line treatment option in advanced age women because of the higher success rates of IVF treatment than COH/IUI treatment.

2.Comparison of The Outcomes of ART Treatment Between Patients < 36 And ≥ 36 Years of Age with Low Serum AMH Levels

INTRODUCTION: To compare the outcomes of ART treatment between women < 36 years and ≥ 36 years of age with low serum AMH levels.
METHODS: Patients who underwent intracytoplasmic sperm injection (ICSI) treatment with low AMH levels at our infertility clinic were enrolled into
the study, retrospectively. Patients were divided into two groups as <36 years and ≥ 36 years of age. The outcomes of ART treatment were compared between two groups.
RESULTS: The total numbers of embryos were found significantly higher in women < 36 years of age than in women ≥ 36 years of age with low AMH levels. The total number of top quality embryos on day 1 was lower in women ≥ 36 years of age than in women < 36 years of
age and the difference between two groups showed statistically significance. The biochemical, clinical, and ongoing pregnancy rates of women < 36 years of age were higher than women ≥ 36 years of age; however only the ongoing pregnancy rate showed statistical significant difference. The cycle cancellation rates of women < 36 years and women ≥ 36 years of age were 1.7% and
15.2%, respectively and the difference was found as statistically significant.
DISCUSSION AND CONCLUSION: The higher ongoing pregnancy rates and embryo quality in younger women is an important outcome to encourage the young women with low serum AMH levels for ART treatment option. Age should take into account in the management and information of women with low AMH level in terms of pregnancy rates and characteristics of ART treatment particularly cycle cancellation rates.

3.Evaluation of 50,465 Cervikal Smear Results in Zeynep Kamil Maternity and Pediatric Research and Training Hospital

INTRODUCTION: Cervical cancer is the second most common type of cancer among women worldwide and considered to be the major cause of death from malignancy in women in developing countries after breast cancer. To detect cervical cancer at very early stages, Pap smear test is used widely.In this study we aimed to evaluate the cervical smear results of the women in our department and to compare with the results of a similar study that had been done for the years 2000-2005 and to discuss in the light of the literature.
METHODS: We performed a retrospective research on 50.465 Pap smear results between January 2012 and February 2016.
RESULTS:
DISCUSSION AND CONCLUSION: The prevalence of cervical cytological abnormalities in our study was %3.4. Many different results were published from Turkey. To determine the real prevalence, more studies with larger numbers are needed.

5.Surgical Treatment of A Huge Submucous Myoma Prolapsed Into The Vagina: Case Report

Nearly 5% of all myomas are originated from the smooth muscle of uterus, beneath the endometrium and called as ‘submucous myoma’. Submucous myomas distort the cavity when they enlarge and may even pass the cervix and reach to the vagina by uterine contractions. The most common treatment of the myomas prolapsing the vagina is the vaginal myomectomy. However, abdominal treatment may be necessary when the submucous myom grow within the cavity and enlarge the uterus. This paper presents a case of huge submucous myoma with a suspician of leiomyosarcoma in the magnetic resonance imaging, that grow into very big size within the cavity and completely fill the vagina. A 45-years-old multiparous women with a complaint of vaginal bleeding was diagnosed to have a 8 cm necrotic submucous myoma prolapsed into vagina after pelvic examination with speculum. Due to the combination of malignancy risk and uterus with three-
month gravida size, abdominal exporation with laparotomy was planned for the patient. In the abdominal exploration, a 14*13*12 cm submucous myoma growing into the endometrial cavity was observed and abdominal hysterectomy was planned.
After completion of abdominal hysterectomy, vajinal part of myoma was tried to extract abdominally but it could not. Due to the complete replacement of vaginal cavity by the myoma, vajinal part of myoma can only be extracted by vaginal approach.

We describe a case of 5-year-old girl who presented with seizures, loss of consciousness, and total loss of vision. Her HSP was atypical because the presenting symptoms were neurological complications rather than the characteristic rash, and her radiological features were compatible with PRES. She did not have renal impairment or hypertension. Her abnormal findings resolved following a standard course of corticosteroids.

7.Lumbar Disc Herniation in Fourteen Years Old Girl

Lumbar disc herniation is a rare disease in children. Children and adolescents with symptomatic lumbar disc herniation have a history of trauma or sports injury before pain begins. There is no difference in the frequency of disc herniation between the gender in children. The segmental placement of disc herniation in children is similar to that seen in adults. A girl aged 14 years presented to our clinic complaining of a one-year low back pain and pain was radiating to the right leg. There was a significant posterocentral disc protrusion at the L5-S1 level on magnetic resonance imaging (MRI). Complete symptomatic relief was achieved at the end of a course of conservative treatment.
